  1. Write down your current housing situation, where you slept last night, and whether you may lose housing within 14 days.
  2. Gather: government ID, DD214 or service information, VA card if you have one, income proof, benefit letters, lease notices, court papers, and any medical or recovery contacts.
  3. If homeless or at imminent risk, call 877-424-3838 (VA) or dial 2-1-1 (Tampa Bay local referrals) first. Ask what VA homeless program or local provider covers your zip code.
  4. Ask specifically about SSVF, HUD-VASH, VA case management, employment referrals, behavioral health, recovery support, benefits help, food access, clothing banks, and transportation barriers.
  5. Keep a simple contact log: date called, agency name, person spoken to, callback number, what was promised, and the next step or deadline.
